Key Responsibilities:
• Provide outreach support to clients in community settings
• Develop intervention and support plans with measurable goals
• Address clients’ daily living needs and health issues




• Maintain comprehensive records and complete monthly reports
• Collaborate with other professionals and families for client care

Requirements:
• Diploma or Certificate in Social Services
• One year of relevant experience preferred
• Ability to pass a Vulnerable Persons Criminal Records Check
• Personal transportation in good repair for client transport
• Excellent written and verbal communication skills
