Be an integral member of the Multi Organ Transplant Outpatient Unit at UHN. In this temporary part-time role, you will measure vital signs, conduct assessments, and perform wound care. Collaborating closely with medical staff, you’ll also prioritize clinic needs, educate patients, and ensure quality care tailored to individual patient goals.

Key Responsibilities:
• Measure and record height, weight, and vital signs
• Conduct basic nursing assessments and collect lab specimens




• Educate patients on health topics and medication reconciliation
• Triage patients and prioritize clinic needs effectively
• Liaise with medical staff and nurse coordinators

Requirements:
• Completion of an approved Practical Nursing program
• Current registration with the College of Nurses of Ontario
• B.C.L.S. certification and medication certificate
• Recent transplant unit experience preferred
• Effective communication and organizational skills
