Health Services – Seniors Services Development
Status: Contract Full-Time (6 Month Duration)
Salary Band: F
Hiring Range: $75,696 – $85,159 per annum Work Mode: Onsite Location: Peel Manor, 525 Main Street North, Brampton Hours of Work: 35 hours per week. Flexibility is required and includes evenings and weekends.

The Seniors Services Development Division is committed to delivering person‑centred care through the Butterfly Philosophy of Care, promoting emotional well‑being and enhancing quality of life for seniors and their caregivers.
While you will not be providing direct resident care, you will play an important role in supporting clients, caregivers, and interdisciplinary teams by coordinating services, facilitating access to resources, and helping older adults remain engaged, connected, and supported in the community.

What You'll Do:
Care Coordination & Client Support
Coordinate client enrollment, scheduling, and case management activities for the Integrated Care Centre (ICC)
Collaborate with clinical and interdisciplinary team members to plan, communicate, and document client care and support services
Maintain collaborative relationships with internal and external partners to support seamless and person‑centred service delivery
Ensure clients and caregivers receive timely information, referrals, and access to appropriate community resources
Coordinate tours and pre‑enrollment trial visits for clients and caregivers accessing the Respite Centre
Leverage community resources and referral pathways to support aging in place and enhance client outcomes
Interdisciplinary Collaboration & Service Delivery
Schedule, assign, monitor, and update interdisciplinary care rounds for Thrive and non‑Thrive clients
Initiate, arrange, and facilitate interdisciplinary care conferences to support client‑centred planning and goal achievement
Communicate with Adult Day Services, Integrated Care Centre, and Respite Centre team members regarding client and caregiver goals, interventions, and outcomes
Support integrated care planning approaches that align services across the circle of care
Administration, Reporting & Quality Improvement
Provide administrative support to the Seniors Health and Wellness Village at Peel Manor and related committees
Coordinate meetings and ensure compliance with applicable legislation, policies, and procedures
Create and maintain electronic medical records within the Telus Collaborative Health Record (CHR) system
Coordinate case management documentation and service delivery activities
Contribute to the collection and reporting of quality measures and performance indicators
Analyze quality outcomes and performance data to support operational planning and continuous improvement initiatives
What You Bring to the Role:
Education & Experience
Baccalaureate degree in a health‑related discipline
Additional education and/or experience in Gerontology would be considered an asset
Three (3) to five (5) years of experience in a health care or community service setting
Experience working with culturally diverse populations
Experience supporting interdisciplinary teams and coordinating client services
Equivalent combinations of education and experience may be considered
Knowledge, Skills & Abilities
Knowledge of age‑related health conditions, the aging process, and person‑centred approaches to care
Knowledge of quality assessment tools, quality improvement methodologies, and performance measurement principles
Ability to assess, collect, interpret, and synthesize information to support planning, evaluation, and service improvement initiatives
Experience utilizing evidence‑based communication and quality assessment frameworks such as SBAR
Strong understanding of Ontario’s Personal Health Information Protection Act (PHIPA)
Experience working within an integrated health care environment and circle of care model
Demonstrated proficiency with Microsoft Office applications and electronic health record systems
Excellent interpersonal, communication, organizational, time management, and decision‑making skills
Demonstrated problem‑solving, critical thinking, conflict resolution, and analytical skills
Ability to respond effectively to changing client and caregiver needs with flexibility and resourcefulness
Demonstrated commitment to diversity, equity, inclusion, and culturally responsive service delivery
Assets
One (1) to two (2) years of experience in case management, service coordination, and/or referral management
Certified Registered Practical Nurse (RPN) designation
Education or experience in Gerontology
Experience working within integrated seniors' health and wellness programs
