As a Wellness and Care Planning Nurse, you will actively support the Director of Wellness in achieving resident wellness goals. Essential skills include proficiency in health record maintenance, clinical assessment, and effective communication with healthcare providers. Your role involves coordinating with Ontario Health at Home and other services to ensure comprehensive care.

Key Responsibilities:
• Assist in creating and maintaining resident wellness goals
• Conduct medication administration audits




• Support the wellness team during absences
• Coordinate in-house physician visits and community services
• Champion health and safety practices within the home

Requirements:
• RPN or higher designation with current registration
• 3-5 years of nursing experience preferred
• Previous supervisory experience required
• Current First Aid and CPR certifications
• Proficiency in MS Office and nursing software
