Become a Youth Supportive Housing Case Manager at CMHA-WECB. This temporary role focuses on providing case management and outreach for individuals with responsible mental illness, fostering community reintegration and stability.

Under the guidance of the Manager, Housing, Facilities, and Employment Services, this position requires adeptness in housing support, mental health advocacy, and crisis intervention. You will construct comprehensive care plans and assist clients in accessing permanent housing solutions. Your efforts contribute to fostering resilience and independence in vulnerable youth aged 15-30.

Key Responsibilities:
• Provide assertive outreach and supportive counseling
• Develop and implement tailored care plans with clients




• Conduct ongoing assessments of client needs and risks
• Facilitate case conferences for client care
• Advocate for necessary resources and services

Requirements:
• Bachelor’s degree in Social Work or Nursing preferred
• Two years’ relevant experience with vulnerable populations
• Knowledge of mental health policies and resources
• Valid Ontario Driver’s License and vehicle access
• Bilingualism in English and French is an asset
