Position Summary:
Reporting to the Clinical Manager of Pharmacy Services youwillbeanintegral member ofthePharmacy teamandbeaccountableforthe pharmacotherapyforpatientsto ensureoptimalpatientcareisachieved.Youwillbeevaluatingevidence-basedmedicinetosupportyourrecommendationsandtodevelopimprovement strategies for drug utilization.
KeyQualifications:
BaccalaureateDegreeinPharmacyorPharm D
Stronginterpersonal,problemsolving,organizational,communication,documentationandcriticalthinkingskills
Experience inprovidingexceptionalpatientcentred care
Experienceinhospitalpharmacysystemsanddistributionsystemsareanasset.
Relevantclinicalexperience preferred
Abilitytoworkinafastpaced environmentbothindependentlyandcooperativelywiththeteam
Demonstratedself-directedlearningandcontinuing education
ResponsibilitiesInclude:
Provisionofcomprehensive pharmaceuticalcaretopatients
Clinicalcareofpatientsincomprehensiveclinicalprograms including warfarindosing, IVtoPO, aminoglycoside and vancomycin dosing.
Incollaboration withotherclinicalpharmacists deliverappropriate teaching topharmacy residents and pharmacy students
Drugdistributionresponsibilities.
Collaboratewithphysicians,nursesandalliedhealthteammemberstoprovideoptimizedpatientcare
OtherdutiesasassignedincludingtheabilitytopotentiallyworkatallTrilliumHealthPartnerssites
Mustbeabletoprovidecoverageonvarious clinicalunitsas needed

Your primary site initially will be Mississauga Hospital; however, you may periodically be required to work at another THP site and therefore periodic travel between sites may be necessary.
