As a skilled Community Nurse, you will deliver compassionate care to ALC clients in residential settings. Your responsibilities include performing comprehensive client assessments, developing personalized care plans, and administering needed medical procedures. Collaborate with multidisciplinary teams to ensure clients receive proper support in their transition of care.

Key Responsibilities:
• Conduct thorough client assessments and create tailored care plans
• Administer medications per established guidelines and document accurately
• Monitor vital signs and report any abnormal findings promptly




• Assist clients with mobility, ensuring safety during transfers
• Maintain accurate and timely documentation of provided care

Requirements:
• Active RN registration with College of Nurses of Ontario
• Current CPR and First Aid certification
• Valid WHMIS certification within last 12 months
• Explicit Criminal Record Check with Vulnerable Sector Screening
• Proven experience in delivering in-home and community care
