Provide individual supports and short-term case management to community members who are homeless or at risk of losing their housing
Facilitate community engagement and outreach
Create housing summits alongside community members to understand local needs, strengths, and opportunities
Outreach and engage residents who are homeless or at risk of homelessness
Support community members in accessing health and social services through a warm transfer process
Develop partnerships with service agencies to ensure efficient and timely access to services
Provide short-term case management and make referrals supporting housing retention
Compile reports on housing needs and community recommendations
Develop and maintain a directory of available housing, supports, and services
Explore and help develop a roommate matching program for community members
Work in Newmarket and across York Region
Requirements Undergraduate degree, preferably BSW/MSW
Minimum 1 year case management/short-term case management experience
Minimum 1 year related experience supporting people with housing needs
Knowledge and appreciation of housing issues in York Region
Knowledge of legislation and regulations, including the Residential Tenancies Act
Knowledge of eviction prevention strategies




Knowledge of community resources and demonstrated ability to search for applicable resources/information
Must be able to work in office and commute across York Region
Robust interpersonal skills, with an ability to work closely with clients and partner organizations
Problem-solving skills in identifying and referring individuals to resources
Core Competencies Demonstrates expertise in case management and community outreach, with a strong understanding of housing issues and resources in York Region. Proficient in developing partnerships and facilitating access to health and social services for individuals at risk of homelessness.
