Responsibilities

- Assess patient/resident/family strengths and needs across physiological, psychological, socio-cultural, and spiritual domains.
- Provide a supportive setting, promote involvement in care decisions, share information, discuss concerns, and respect personal choices.
- Contribute to developing and updating individualized plans of care; participate in multidisciplinary care planning meetings and family conferences.
- Deliver nursing care according to policies, procedures, technology, and FHA standards; document observations and communicate changes to designated staff.
- Administer admissions, discharges, and transfers following agency policies and procedures.




- Document and update care information to ensure patient safety and comfort.
- Operate and maintain equipment related to patient care (e.g., bathtubs, scales, mechanical lifts).
- Maintain a safe and clean environment for patients/residents and staff.
- Assist in orienting new staff, students, and other Health Care Team members.
- Participate in quality improvement activities, education sessions, team conferences, meetings, and committees.
- Perform other related duties as assigned.

Qualifications

Education and Experience

Graduate of a recognized practical nursing program or equivalent combination of education, training and experience. Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).

Skills and Abilities

- Ability to communicate effectively, both verbally and in writing.
- Ability to work with others effectively and to organize work and implement a plan of care.
- Knowledge and competencies to work at full scope of practice as required by BCCNM, including administering medications.
- Ability to operate related equipment and physically carry out the duties of the position.
