This is a community leadership role.The RoleWe are hiring a Community Health Representative (CHR) to lead growth and community engagement in Burnaby and New Westminster.This position is ideal for someone who:Loves being on the road and building real relationshipsIs confident conducting in-home assessmentsThrives in independent, performance-based environmentsUnderstands that trust is built face-to-faceYou will spend approximately:90% of your time in the field visiting referral partners and meeting families10% of your time in your home office updating CRM and managing follow-upsWhat You'll Be Responsible ForCommunity Relationship DevelopmentBuild and maintain referral partnerships with occupational therapists, hospital discharge planners, social workers, physicians, pharmacists, assisted living sites, and other healthcare professionals.Represent Just Like Family at community events, networking meetings,
