ROLE The Regional Clinical Planning and Transformation Lead will lead through influence by coordinating existing regional initiatives, planning tables, leadership groups, and organizational resources to advance clinical system transformation. The role will identify opportunities to align priorities, reduce duplication, share expertise, and leverage existing capacity across organizations.

Working closely with executive sponsors, hospital leaders, physicians, Ontario Health, the HPA-OHT, municipalities, and community partners, the Lead will support collective planning, co-design, implementation readiness, and regional decision-making while ensuring that work is embedded within existing organizational and governance structures wherever possible.

The role facilitates options, alignment, implementation readiness, and decision support; it does not make decisions on behalf of hospital executives, medical leadership, participating organizations, or boards.

The Lead will facilitate the development and assessment of options but will not replace the decision-making authority of participating organizations, executive sponsors, medical leadership, or boards.

REQUIRED EDUCATION & CERTIFICATION(S)

- Post-graduate degree or equivalent experience in health administration, business administration, public administration, project management, health system planning, health policy, or a related field




- Change Management / Project management qualification (PP, CAP, Project+) or equivalent work experience (preferably healthcare and/or population health related)

REQUIRED SKILLS & EXPERIENCE(S)
- Minimum 3–5 years of experience in healthcare project management, system planning, clinical operations, quality improvement, change management, health system transformation, or regional clinical services planning.
- Experience working in complex, multi-site or multi-organization healthcare environments.
- Experience facilitating clinical planning, quality improvement, Lean, redesign, integrated care initiatives, executive planning tables, or sensitive multi-party discussions.
- Experience supporting executive committees, governance tables, boards, regional planning processes, and options assessment.

DESIRED SKILLS, EXPERIENCE(S) & EDUCATION
- Project Management Professional certification, change management certification, quality improvement training, facilitation or mediation training, or equivalent experience would be considered an asset.
- Existing clinical project manager qualifications identify a post-graduate degree, PMP certification or equivalent project management experience, and healthcare/business administration experience as preferred assets.
- Rural health system experience and/or Ontario Health Team experience would be an asset.
