The Assertive Community Treatment (ACT) Team, a best practice model providing specialized community care supports people living with severe and persistent mental illness in achieving satisfying, hopeful, contributing lives. By acting as the clinical and administrative supervisor for the team, the ACT Coordinator ensure that ACT services demonstrate excellence, utilizing evidence-based practices guided by recovery principles to achieve fidelity to ACT Standards. The ACT Coordinator directs the multidisciplinary team in conjunction with appropriate psychiatric support to ensure service excellence and courteous, helpful, and respectful services to program clients; and may be required to function as a practicing clinician on the team.

Duties

- Support the day-to-day clinical and administrative operations of the ACT team including ensuring appropriate coverage for day, evening, weekend, and holiday shifts.
- Lead daily organizational staff meetings and ensure that daily assignments are organized in an effective and efficient manner.
- Lead Treatment Planning meeting and ensure Treatment Plans are being updated as scheduled and required.
- Direct and coordinate the client admission process and treatment, rehabilitation, and support services in coordination with the psychiatrist.
- Direct and coordinate comprehensive client centered assessments and individualized treatment planning by working closely with clients and team members.
- Advocate for interventions supporting improved client outcomes.
- Continuously evaluate the status of clients and provide appropriate planning and coordination of treatment activities to ensure immediate attention to changing needs.
- Carry a small caseload of clients.
- Provide support to Early Psychosis Intervention program.
- Provide support and education to ACT clients and family members.
- Provide education and in vivo supervision to assist staff in developing goals and required skills for position.
- Monitor medical records management assuring maintenance of the medical record in compliance with agency policies; train staff on medical record requirements, regularly review client assessments, Treatment Plans, and progress notes written by the staff; Provide feedback to individual staff to ensure medical records proficiency.




- Liaise with community organizations, employers and government officials for purposes such as services, employment and housing activities.
- Deliver education and consultation to clients, family members, hospital departments, community partners and other participants to promote ACT services.
- Maintain regular communication with program leadership regarding operational activities, emerging issues, and key developments within the office workplace.
- Support program fidelity monitoring of the SAH ACT team.
- Participate and adhere to any quality assurance practices and research initiatives as set out by the Ministry of Health and as otherwise determined or assigned.
- Participate in staff recruitment, interviewing (as subject matter expert), work assignments, orientation, and supportive conversations with team members. Assist in identification of performance challenges.
- Provide input for creation and updates of program policies.
- Assist the Mental Health and Addictions leadership team in understanding internal and external trends with respect to the clinical programs’ specialization and to develop and maintain effective links with other clinical/service programs.
- Participate in Accreditation process.
- Other duties as assigned.

Qualifications

- Masters level prepared
- Registration with one of the following governing bodies:
- College of Registered Psychotherapists of Ontario
- College of Occupational Therapists of Ontario
- College of Psychologists of Ontario
- Ontario College of Social Workers and Social Service Workers
- College of Physiotherapists

- Demonstrating understanding of Mental Health Act and health system navigation.
- Valid G driver’s license and access to a vehicle

Experience





- Three to five years’ experience in a clinical or community setting providing direct services to clients with severe and persistent mental illness.

- Preference will be given to individuals with a minimum of 1 year of demonstrated formal or informal leadership experience in a healthcare setting.

Knowledge And Skill

- Clear understanding of the characteristics and problems of adults with severe and persistent mental illnesses
- Knowledge of the Ontario ACT and Early Psychosis Intervention Program Standards
- Experience providing services to persons with severe and persistent mental illness within the context of a multi-disciplinary team.
- Knowledge of multiple issues pertaining to persons with severe and persistent mental illness, including systemic barriers.
- Extensive knowledge of community resources.
- Strong computer skills are required, including proficiency with Microsoft Office applications (Word, Excel, Outlook, and Teams), electronic documentation systems, and the ability to learn new software quickly.
- Ability to work effectively and efficiently as a team member in a fast-paced environment in stressful situations.
- Ability to maintain successful and respectful working relationships with peers and the interdisciplinary team to achieve positive patient outcomes and promote a positive team environment.
- Exhibited exceptional relationship-building skills through empathy, compassion, and an understanding of human and organizational dynamics, while employing motivational and counseling strategies to support patient learning, engagement, and therapeutic goals.

- Experience providing consultation to other multi-disciplinary team members from a variety of backgrounds.
