The incumbent will be required to work field hours and may be required to work into the evening. Working in a multidisciplinary setting including, but not limited to, Primary Health Care Team members: Family Physicians, Mental Health Counselor, Home Care, Therapy services, Public Health, Dietitians, Healthy and Home, Psychiatrists, Medical Office Assistants and other team members as this position requires networking with other professionals and agencies within the community.Human Resources Exemption:
