Community Outreach Worker - Urgent Service Access Team (USAT) –Contract , Full Time, Quest Community Health Centre The Community Outreach Worker in the Urgent Service Access Team (USAT) is responsible for providing outreach, health education and administers and implements a variety of social assistance programs and community services, to assist clients to maximize the social determinants of health. The position has a strong emphasis on health promotion, disease prevention, and community capacity building and works within a harm reduction framework. These duties are accomplished as part of an interdisciplinary team of professionals whose objective is to provide primary health care and follow-up for clients.

QUALIFICATIONS INCLUDE:
Successful completion of a college or university program in social work, child and youth care, counseling or other social science or health-related discipline.
2 to 4 years experience in a community-based social service setting
2 to 4 years providing assessment and brief intervention counselling is an asset.
QUEST CHC OFFERS:
Salary Range: $66,292.68 to $82,401.88 annually
Plus: Optional HOOPP (Pension Plan)

Employment Status: 6 month Contract, Full time - Opportunity for renewal
Hour: 37.5 per week - The candidate must be able to work varying work schedules (typically 11:30am to 7:30pm), including evenings, and in multiple work locations. Flexibility is required, as hours may vary occasionally to accommodate organizational events, staff training, and meetings.
