As an LPN at our facility, you will provide professional practical nursing services to designated patients within a client-centered care model. The LPN will work under the general supervision of the Director of Care and collaborate with the healthcare team to ensure high-quality patient care.Duties:- Provide independent nursing services and administer prescribed medical treatments to patients.- Collaborate with healthcare team members and participate in coordinating patient care.- Practice within the scope of LPN standards and guidelines.- Implement nursing care plans in conjunction with residents, families, and other healthcare professionals.- Document nursing care accurately using the nursing process.- Maintain communication with the healthcare team to ensure comprehensive patient care.- Report any changes in patient status promptly to the appropriate healthcare professionals.- Uphold professional standards of behavior and appearance.- Demonstrate excellent communication and interpersonal skills in all interactions.Requirements:Education/Experience:- Graduate of an approved Licensed Practical Nursing Education program.- Current registration with the College of Licensed Practical Nurses.- Experience in geriatric care is preferred.Qualifications and Skills:- Knowledge of LPN standards, code of ethics, and scope of practice.- Familiarity with facility policies,
