Position Summary: Reporting to the Mental Health Program Manager, two positions will support the Nurses’ Health Program (NHP). The NHP is a voluntary recovery monitoring program for Ontario nurses with substance use and/or mental health disorders that may affect their ability to practice nursing safely. The NHP monitors nurses’ recovery so they may be supported to practice safely or return to practice, which promotes professional accountability and protects the public. The role includes maintaining a caseload of nurse participants through their recovery plan, acting as guide and central point of contact for participants to successfully fulfill their agreed upon monitoring contract. This role involves liaising with multiple stakeholders, program participants, community supports and physicians. The NHP Case Manager will ensure the timely submission of required reports, as well as support NHP functions, including Directorate and Clinical Advisory Panel meetings. As a member of a dynamic interdisciplinary team of professionals, your expertise in this field will enable you to contribute to discussions regarding workplace disability‑related issues. NHP Case Manager Responsibilities: The NHP Case manager is responsible for initiating, engaging, and maintaining contact with the NHP members on a regular basis for the duration of their contract with the NHP. Provide support to employee manager/supervisor and client HR team regarding case management, disability, and accommodation. Interpret and educate the client’s managers on a variety of regulatory and compliance requirements to ensure that case management practices are applied in accordance with legislation, regulation and client policies. Collaborate with community providers to ensure appropriate resources are accessed, such as EFAP providers, specialized assessments, or specialized treatment programs.



Collaborate and liaise with other members of the internal team and the members’ Health & Safety team. Act as liaison, mentor and coach regarding disability claims to both internal and external partners. Collaborate with relevant stakeholders (e.G., employer, union, regulatory college) to facilitate effective and sustainable return‑to‑work plans. In collaboration with the client’s managers and human resources business partners, regularly monitor and adjust the progress of all return‑to‑work cases to resolve emerging issues. Develop effective relations with the employee as well as the client’s human resources, management, union and relevant health care professionals to implement return‑to‑work and/or modified work programs. Provide education and training to the client’s managers and staff regarding the disability case‑management processes. Maintain and respect confidential information, objectively and accurately document case history, strategy and case‑management plan. Credentials and Professional Training: Regulated health profession, in good standing with one of the professional colleges in Ontario. Current registration as a Registered Nurse in positive standing with the College of Nurses of Ontario. Bilingual in French and English preferred. Key Qualifications: Certified Disability Management Professional or Certified Return to Work Coordinator credential is preferred but not mandatory. Minimum of 2 years of experience in disability case management. Demonstrated expertise and experience with return‑to‑work planning, long‑term disability contracts, collective agreements,



and attendance and accommodation programs in a large and diverse organizational environment. Working knowledge of relevant legislation (e.G., Ontario Human Rights Code, Occupational Health and Safety Act). Knowledge of the Workplace Safety and Insurance Act is an asset. Excellent interpersonal skills are required. Excellent communication, organization, and presentation skills. Excellent computer skills (Word, Excel, HRIS); able to create and generate reports as required. Superior assessment and problem‑solving skills. Location: The Disability Case Manager will be working at 2381 Bristol Circle, Oakville or at the West Toronto location at 150 Sherway Drive as directed by THP at its sole discretion, though there may be some opportunity for flexible work environments.
