About Southridge Shelter:
Southridge Community Church operates the Southridge Shelter - a 24/7, housing-focused emergency shelter committed to offering compassionate, progress-oriented support to individuals experiencing homelessness. We provide a welcoming and inclusive environment where all guests are treated with dignity and respect. We see the shelter as a short, supportive stop on each person’s path toward positive housing. Supporting that journey requires both genuine care and thoughtful accountability.
Purpose of this role:
To provide housing-focused case management within our shelter workplace, supporting residents to secure and maintain permanent housing as quickly as possible. The Shelter Coach works alongside residents to develop and implement individualized, goal-oriented plans that prioritize housing stability, while responding to immediate needs with dignity, respect, and a strengths-based approach.
Respnsibilites:
· Lead intensive, individualized case management for an assigned caseload of residents.
· Work collaboratively with residents to develop clear, action-oriented housing plans, including defined goals, timelines, and responsibilities.
· Support residents in navigating housing options
· Monitor progress through regular case reviews, adjusting plans and reinforcing accountability.
· Prepare residents for successful tenancy through budgeting, landlord relations, and skills required for maintaining housing.
· Coordinate and support successful transitions out of shelter, including warm handoffs to community supports.
· Act as a key connector between residents and the broader homelessness and housing system.
· Build and maintain strong working relationships with: Housing providers and landlords, Community agencies and Health, mental health, and addiction support organizations
· Participate in case conferencing and coordinated care planning as required.
· Advocate for residents to reduce barriers to housing and services.
· Build supportive, respectful relationships with residents, recognizing their strengths and potential.
· Provide support that reflects a trauma-informed, client-centered, and non-judgmental approach.
· Respond to crisis situations with appropriate de-escalation and problem-solving strategies.
· Encourage resident engagement in their own goals, reinforcing accountability and empowerment.
· Support a positive shelter environment that fosters community, dignity, and belonging.
· Maintain accurate, up-to-date case notes and documentation in accordance with organizational and Regional requirements
· Track and report on housing outcomes, length of stay, and case management activities
· Participate in team meetings, case reviews, and continuous improvement efforts.
· Be guided in data collection by our By-Name Data health.
· Be trained in and complete data reporting through HIFIS.
· Ensure confidentiality and ethical handling of all resident information.
· Be trained in full Diversion tool in order to support in peak times
· Provide support using Diversion and My Re-Housing Plan for early engagement in a resident’s stay.
· Follow all organizational policies, procedures, and health and safety standards
· Contribute to a safe, respectful, and inclusive shelter environment

Pay: $50,724.00-$56,692.00 per year
