In this critical role, you will report to the Home Care Manager and collaborate with the Registered Nurse Case Manager.

Your responsibilities will include using your assessment skills to coordinate quality care, supported by your nursing education and experience. You'll function as part of an interprofessional team, delivering patient-centered services in various settings. Key Responsibilities:

- Assess client needs while consulting with the Registered Nurse
- Plan and coordinate individualized care plans
- Provide direct nursing care in clients' homes
