: The CDM Clinical Nurse Educator reports to the Manager Clinical Integration and works with an interdisciplinary team with a chronic disease prevention and management focus. The CDM Clinical Nurse Educator integrates in-depth clinical expertise, experience, and knowledge with the principles of teaching and learning, in the provision of chronic disease self-care management for clients, families, health professionals and students.

- 3 years clinical experience with patients/families with chronic disease
- Basic - Computer skills
- Advanced - Interpersonal skills
- Basic - Assessment skills
- Advanced - Communication skills
- Basic - Facilitation skills
- Demonstrates word processing skills using Microsoft Office programs
Knowledge and Abilities
- Ability to develop, evaluate and modify programming provided to clients
- Ability to adapt to changing conditions
- Communication skills to transfer knowledge/skills
- Ability to develop and deliver formal education programs
- Ability to competently use computers and software applications
- Demonstrates ability in teaching, facilitation and group activities
