Our Indigenous Primary Care Organization is dedicated to improving the health and wellbeing of Indigenous peoples, both in our local First Nation communities and those living in Urban and rural centres throughout Lambton County. Our team provides wholistic primary care that integrates clinical excellence with Indigenous knowledge, culture and traditions.

Position Summary

E-nangaabe-jig Health Services is seeking a compassionate and skilled Nurse Practitioner (NP) to join our growing interdisciplinary primary health care team. The Nurse Practitioner will provide comprehensive primary care services to Indigenous patients and families in a culturally safe, community-centered environment.

Working within a wholistic model of care that respects Indigenous traditions, values, and approaches to wellness, the NP will focus on health promotion, chronic disease management, preventive care, and patient education, while supporting improved health outcomes for First Nations, Inuit, and Métis communities.

The Nurse Practitioner will work collaboratively with physicians, nurses, traditional healers, community health workers, and other allied health professionals to deliver integrated care that addresses the physical, mental, emotional, and spiritual wellbeing of patients.

Key Responsibilities

Clinical Care

- Provide comprehensive primary health care including assessment, diagnosis, treatment, and follow-up for patients across the lifespan.
- Order and interpret diagnostic tests and laboratory results.
- Prescribe medications and therapeutic interventions in accordance with provincial regulations.
- Provide preventive care including screening, immunizations, and health promotion.
- Manage acute illnesses and chronic diseases including diabetes, hypertension, respiratory disease, and cardiovascular conditions.
- Deliver culturally safe and trauma-informed care that reflects the needs of Indigenous patients and families.

Chronic Disease Prevention & Management

- Lead initiatives supporting diabetes prevention and management, cardiovascular health, and other chronic conditions common in Indigenous populations.
- Provide patient education and self-management support.
- Collaborate with community programs addressing physical, spiritual and social determinants of health.

Community & Cultural Engagement

- Work respectfully in allyship with Indigenous communities, Elders, and knowledge keepers.
- Incorporate Indigenous perspectives of health and wellness where appropriate.
- Participate in community outreach and health promotion initiatives.

Interdisciplinary Collaboration

- Work as part of a team-based primary care model alongside physicians, traditional healers, cultural practitioners, nurses, social workers, dietitians, and other health professionals.
- Participate in case conferences and collaborative care planning.
- Support coordination of care and patient navigation across the health system.

Documentation & Quality Improvement

- Maintain accurate and timely patient records using electronic medical record.
- Participate in quality improvement initiatives and data reporting as required.




- Contribute to organizational policy development and clinical protocols as required.

Qualifications

Education & Licensing

- Registered Nurse in the Extended Class (RN-EC) in good standing with the College of Nurses of Ontario (CNO).
- Completion of a Master's Degree in Nursing, Nurse Practitioner program from an accredited institution.

Experience

- 3-5 years of clinical experience.
- Experience in primary care, community health, or Indigenous health settings is considered an asset.
- Experience managing chronic disease, particularly diabetes, is an asset.
- Demonstrated experience working within interdisciplinary health teams.

Knowledge & Skills

- Strong understanding of Indigenous health issues, social determinants of health, and culturally safe care.
- Knowledge of trauma-informed and patient-centered care approaches.
- Excellent communication, clinical decision-making, and relationship-building skills.
- Ability to work independently while collaborating effectively with a multidisciplinary team.

Other Requirements

- Valid Ontario driver's license and access to reliable transportation may be required.
- Ability to obtain a satisfactory vulnerable sector check.
