New or Replacement Position: Replacement Site: Princess Margaret Cancer Centre Department: Oncology Nursing Leadership - Collaborative Academic Practice Reports to: Director of Professional Practice, Oncology and Blood Disorder Programs Wage Range: $66.40-$79.27 per hour (start rate in accordance with collective agreement) Temporary Full-Time (12 months) The Nurse Practitioner (NP) in the Pediatric Cancer Survivorship program will function in an advanced nursing role, demonstrating a high level of autonomy and expert skill to formulate clinical decisions and provide management and surveillance for late effects of childhood cancer. They will provide outpatient care for patients with a history of childhood cancer, including brain tumors, solid tumors, and leukemia/lymphoma. Work with Pediatric Cancer Survivors in 2 Survivorship Clinics Utilize the Children’s Oncology Group guidelines to create surveillance recommendations and follow-up plans Disseminate recommendations and plans to primary care providers Be responsible for organizing surveillance investigations,



following up on results and initiating referrals to specialists Collaborate with the SickKids Aftercare Team and attend 2 Transition Clinics each year Assist with reviewing referrals from other providers to access the Survivorship Clinic and be responsible for referring patients to other Survivorship Clinics across Canada when needed Work primarily in-person with the occasional virtual clinic visit Other responsibilities will include academic and leadership activities, including delivering, coordinating, and participating in research, education and professional development activities.

Completion of a Master’s degree in Nursing required Current registration with the College of Nurses in Ontario in the Extended Class Experience with the Adolescent and Young Adults (AYA) population Experience in quality improvement and program development preferred Proficiency in information technology and virtual care platforms Canadian Association of Nurses in Oncology) As a Nurse Practitioner in the Brain Metastases and Gamma Knife program, you will have the opportunity to make a significant impact on the lives of patients and families while working in a world-class cancer centre.
