Your Opportunit y:The Quality Improvement Decision Support Specialist (QIDSS) is responsible for supporting primary care practices across the Hamilton Family Health Team (HFHT). Facilitates development and implementation of quality improvement plans/initiatives, and engaging teams in the use of data to support quality improvement goals and clinical process chang e.

Roles and Responsibiliti es

• Collaborate closely with teams engaged in quality improvement to identify, performance measurement information and data quality nee ds,• Support the development and implementation of quality improvement pla ns,• Support decision making by developing, generating, analyzing and interpreting extracted health informati on,• Ensure high quality data is available for extraction by developing and implementing systems to identify, tract, correct and prevent erro rs,• Identify gaps in data availability, and define and implement solutions to close the ga ps,• Provide consultative and analytical support regarding the collection, interpretation, analysis and presentation of data to tea ms,• Act as a change agent and coach to teams on data management and performance measureme nt,• Provide guidance and support to teams regarding the collection, interpretation,



analysis and presentation of data to improve patient care and align with best practi ce,• Facilitates the development of effective working relationships with diverse tea ms,• Take leadership roles in connecting teams and spreading best practic es,• Contribute actively to local and provincial quality improvement initiativ es,

Qualifications and Ski lls

• A university degree in a related discipline (E-Health /Health Informatics, Business Administration, Statistics, Health Sciences or Information Management), where possible combined with clinical experience in primary care or equivalent education and experie nce,• Highly effective communication, presentation and interpersonal skills and a proven ability to develop and maintain strong relationships with partners, Working knowledge of adult education theory and meth ods,• Experience creating a positive and inclusive learning environm ent,• Understanding of the importance of vision and enthusiasm in creating and promoting cha nge,• Knowledge of primary care practice and demonstrated interest in the use of data for quality improvem ent,• Ability to handle a variety of concurrent assignments and to work independen tly,• Valid Ontario driver’s lice nse,• Strong applied knowledge of one or more EMR/HER systems currently in use in Ontario and willingness to become proficient in other syst ems,• Demonstrates proficiency in programming in a language used to extract data from EMR (e.g. SQL) is requi red,• Knowledge of additional report writing and statistical analysis packages (SSRS, SSIS, SSAS or Crystal Reports )
