Case Manager – Short Stay Crisis Beds Program (Full-time Contract – 3 Months) The Role and Opportunity The Canadian Mental Health Association Toronto offers intensive case management services as part of the continuum of mental health services and supports for people with mental illness. We are currently seeking a full time contract Case Manager for our Short Stay Crisis Beds program. The program provides 24‑hour on‑site residential support and short‑term case management services to individuals living with mental illness who have been involved with the criminal justice system. The Case Manager will offer onsite support to clients as well as community‑based services and interventions. Responsibilities Program outreach and assessment Orientation to program, agency and community Client‑centered engagement, service provision and interventions Crisis prevention and intervention Skill development and goal planning Providing resources, rights information and advocacy Maintaining a trusting, respectful and collaborative working relationship Monitoring, evaluation and follow‑up Accurate, timely documentation Some coordination of site maintenance duties may be required Skills You Bring Minimum undergraduate degree/diploma in Social Work/Social Science or related field A minimum of 3 years full‑time experience in direct service delivery to adults with psychiatric disabilities Strong assessment, service planning and crisis intervention skills Work experience and knowledge of concurrent disorders, homelessness,



recovery principles and the criminal justice system as it relates to mental health Strong organizational and time‑management skills Ability to work collaboratively in a team environment Excellent communication, listening and interpersonal skills Ability to demonstrate an anti‑oppression approach to practice A valid Ontario driver’s license is required.
