Responsibilities Reporting to the Manager Home and Community Care and in collaboration with the Supervisor of Health Programs (SHP) and the Community Health Nurses (CHN), the Supervisor Home and Community care is responsible for providing nursing care and support services to the general population throughout the life cycle to enable them to stay in their homes and to promote optimal level of functioning in activities of daily living.
