Role Overview

Consult with internal and external health care professionals on the application of advanced theory to the delivery of pharmaceutical care.

Provide clinical direction on complex cases using evidence‑based evaluations of drug therapies and patient medical histories.

Participate in patient education and discharge consultations, providing recommendations in the planning and treatment assessment policies and procedures.

Assist in the development of drug therapy guidelines for individual disease conditions.

Identify and initiate clinical drug research.

Prepare case studies and articles for submission to professional journals.

Conduct pharmacological education or in‑service sessions.

Represent the department on multidisciplinary care committees, and at external events.

Additionally, we would rely on your expertise to ensure that our pharmacists have the most current information as they perform vital works such as validating prescriptions and identifying safety concerns.

Required Qualifications

Current registration as a Pharmacist with the College of Pharmacists of British Columbia.

Completion of an accredited Hospital Pharmacy Residency Program.

Post‑graduate Doctor of Pharmacy degree.

One (1) year of pharmacy experience in the designated specialty area.

Detailed Overview
Working within a collaborative team environment, the Clinical Pharmacy Specialist functions as an expert practitioner, consultant, educator, and researcher in a designated specialty area. The role provides consultation services to internal and external health care professionals and applies advanced theory to assess, plan, implement, and evaluate pharmaceutical care delivery. The Specialist develops, plans, organizes,



and evaluates clinical, educational, and research activities, including teaching undergraduates, graduates, and pharmacists, and participating in medication use evaluation programs, treatment policies, continuing education, literature reviews, and clinical drug research initiatives.

Responsibilities

Provide consultation services and apply advanced theory to assess, plan, implement, and evaluate pharmaceutical care in the designated specialty area.

Provide clinical direction on complex cases, attend patient care rounds, review medication histories, provide drug information, patient education, and discharge consultations, and recommend policies and procedures.

Ensure quality assurance programs and policies are maintained, troubleshoot inconsistencies, develop standards, conduct audits, and take corrective action as required. Participate in quality improvement activities and prepare reports.

Provide academic teaching in the designated specialty area to undergraduate and graduate students, including curriculum planning and practicum coordination.

Identify and initiate clinical drug research, develop hypotheses, oversee research activities, analyze data, and publish findings.

Conduct pharmacological education and in‑service sessions, and orientation sessions for pharmacy staff and other health care staff.

Participate in the Pharmacy and Therapeutics Committee, preparing materials for formulary decisions and policy making, evaluating formulary requests, and presenting drug assessments.

Participate in planning, implementation, and evaluation of therapeutic cost‑effectiveness and cost‑restraint programs, and recommend new or revised policies.

Collaborate with program team members to develop, implement, and evaluate goals, objectives, and outcomes for the program, including policy recommendations.

Represent the department on multidisciplinary care committees and at external events such as conferences and regional planning groups.

Provide pharmaceutical care to patients in the designated specialty area, review charts, identify and solve drug‑related problems, monitor therapy, maintain medication profiles, and educate patients and families.

Perform other related duties as assigned.

Qualifications – Education and Experience




Current registration as a Pharmacist with the College of Pharmacists of British Columbia.

Certificate of completion of Year 1 and Year 2 of a Hospital Pharmacy Residency Program accredited by the Canadian Pharmacy Residency Board (CPRB) supplemented by two (2) years of recent, relevant hospital pharmacy experience, including one (1) year in the designated specialty area or an equivalent combination of education, training, and experience.
