In this position within PHC's Urban Health & Substance Use Program, you'll work across various clinics, including the Lighthouse Substance Use Care Clinic.

Assess and document patient health status and needs
Collaborate to develop and assess care plans

Participate in ongoing staff training and development Requirements:
Registration as a Registered Nurse or Psychiatric Nurse
