As an Outreach Support Mentor, you will work closely with the Outreach Team, including Case Workers and the Clinical team, to ensure program requirements are effectively met. Key responsibilities include planning activities, managing participant files, and maintaining high standards in service delivery. Your role will also involve limited administration while ensuring positive experiences for program participants.

Key Responsibilities:
• Provide direct support to Outreach program participants




• Collaborate with the Outreach Team and Clinical staff
• Assist in planning and monitoring activities
• Ensure compliance with health and safety policies
• Maintain accurate program documentation

Requirements:
• Previous outreach experience required
• At least two years in a related role
• Certification in First Aid Level 1
• Satisfactory background checks
• Reliable vehicle and driver’s license
