As a Social Worker at Bethany, you will ensure quality, resident-centered care through assessment, guidance, and education. Engage with interdisciplinary teams to advocate for residents, promote choices, and facilitate independence while addressing their psychosocial needs. You'll document assessments meticulously and participate in care planning to enhance discharge readiness and resource navigation.
Key Responsibilities:
• Conduct detailed psychosocial assessments for residents and families
• Provide individualized interventions adhering to ACSW Standards
• Facilitate therapeutic support groups for residents and caregivers
• Collaborate with the interdisciplinary team on care planning
• Advocate for resident access to community resources
Requirements:
• Bachelor’s in Social Work required
• Two years of relevant Social Work experience, preferably in gerontology
• Registered with the Alberta College of Social Workers
• Robust interpersonal and communication skills
• Ability to work independently with minimal supervision
