Perform psychosocial assessments and interventions
Work with interdisciplinary teams for holistic care
Help patients transition from hospital to home safely
Connect individuals to essential community resources
Strengthen coping strategies for chronic health issues Requirements:
Bachelor’s Degree in Social Work required
Two years of recent experience in healthcare
Current registration with British Columbia College of Social Workers
Valid BC driver’s license is a must
