Your role will focus on providing research-based support, creating connections with community resources, and fostering family-directed goals. Flexibility in scheduling is essential including the possibility of evenings and weekends.

Key Responsibilities:
• Establish trusting relationships with families through home visits
• Support families in their goal development and objectives




• Assist in referral processes for community supports
• Maintain accurate records of home visits
• Work both independently and effectively as part of a team

Requirements:
• Grade 12 education or equivalent
• Experience with children and families is an asset
• Valid driver's license required for home visits
• Clear Criminal Record Check within the last 6 months
• Ability to adapt to diverse family needs
