Key Responsibilities:
• Assess needs of individuals and families at risk of homelessness
• Connect clients with appropriate housing and community supports
• Conduct follow-ups to ensure ongoing support and stability

• Maintain accurate records of client interactions and progress
Requirements:
• Relevant experience in social services or community support
• Solid communication and interpersonal skills
