Join Carefirst FHT as an Integrated Primary Care Coordinator focused on enhancing patient attachment to primary care. Collaborate within a community-focused team to connect individuals with essential health services.

The position involves working closely with Ontario Health Team partners and engaging with the community to establish ongoing primary care connections. With a primary responsibility to identify unattached patients, you'll leverage excellent communication and interpersonal skills to improve collaboration among various health care organizations. Your efforts will play a vital role in navigating primary care resources for the community.

Key Responsibilities:
• Develop strategies to identify unattached patients
• Collaborate with community service providers




• Coordinate outreach and engagement events
• Support primary care practices with follow-ups
• Maintain reporting for primary care projects

Requirements:
• 5 years relevant health experience preferred
• Undergraduate degree in health-related field
• Experience with physician engagement
• Solid knowledge of Ontario health care
• Excellent communication skills, both verbal and written
