This contract role focuses on providing direct support for youth at risk of homelessness in Scarborough. Collaborate with youth through a strengths-based approach to develop effective housing alternatives. You will facilitate case planning, implement diversion assessments, and connect clients with community resources enhancing their engagement.
Key Responsibilities:
• Assess youth entering the shelter for safe living options
• Coordinate case management and support planning
• Develop diversion assessment tools with evaluation collaboration
• Provide conflict resolution and mediation services
• Maintain professional documentation of interactions
Requirements:
• Undergraduate Degree in Social Work or related field
• 2+ years’ experience in youth social services
• Knowledge of trauma-informed care practices
• Solid communication and data literacy skills
• Police Record Check and medical clearance required
