247 East Avenue N, Hamilton, ON L8L 0E1, CanadaRegistered Social Worker – Full Time - 14 month contractHamilton, ON - The OaksJob DescriptionReporting to the Program Supervisor, as a Registered Social Worker you will work within a collaborative interdisciplinary team, coordinating and delivering mental health and addiction services to tenants experiencing long‑term housing instability and/or institutional stays. The focus is to assist them in transitions to/from hospital, incarceration, treatment, and intake into Indwell’s program to help achieve health and housing stability. This role requires resilience and ability to work near challenging situations.Hours of WorkFull time 14 month contract, 40 hours per week, 8 hours a day. Primarily Monday‑Friday during regular business hours, but schedule may include evenings, weekends, and public holidays. On‑call for one week every 4‑6 weeks.ResponsibilitiesBuild therapeutic relationships with tenants and apply a trauma‑informed, hope‑promoting approach.Screen, intake, conduct substance use assessments, complete and refer.Collaborate with internal and external partners, including medication administration, apartment checks, community events.Assess complex situations and make decisions independently and as part of a team.Utilize crisis management and intervention skills.Develop and facilitate relevant group programming.Support tenants with health, housing, self‑management, and system navigation.Compile community resource information and lead workshops.Participate in on‑site and in‑home assessments and interventions.Interdisciplinary SupportPartner with multi‑disciplinary staff for shared decision‑making and case management.Utilize asset‑based and person‑centered frameworks to create and manage care plans.Advise tenants on community services, health promotion,



self‑advocacy.Facilitate information sessions and workshops for tenants.Assessment and DocumentationParticipate in intake process and assessments.Lead coordinated care planning with internal and external supports.Complete documentation thoroughly and timely.Perform assigned administrative tasks such as rent collections.QualificationsBachelor of Social Work (BSW) degree or equivalent.Registered with a regulated college of professionals or willing to register.Experience with individuals experiencing serious mental illness, addictions, or special needs.Knowledge of serious mental illness, addictions, concurrent disorders, dual diagnosis, and relevant legislation.Familiarity with community and social services in the St Thomas area.Strong interpersonal skills, flexibility, organization, initiative, problem‑solving, and genuine interest in supporting people with mental illness and addictions.Harm reduction and trauma‑informed lens.Recent Police Vulnerable Sector Check accepted by Indwell.Current Standard First Aid & CPR Certification or willingness to register within 30 days.Experience supporting diverse communities and commitment to cultural safety, anti‑racist and anti‑oppressive practice.Strong leadership skills, critical thinking, and mentoring passion.Valid “G” class driver’s license and own vehicle.BenefitsVacation: 3 weeks paid with 8 HEAL days (sick and personal appointments) per year.Extensive benefits: Extended health (100% medication coverage, some exceptions may apply), dental and vision care, life insurance and long‑term disability.Employee Assistance Program: primary care and qualified consulting services.Group RRSP: contributions amounting to 5% of salary.New Staff Orientation Days.Professional and personal development: annual Leadership Conference, Spiritual Ranch Retreat, Cliffs Strengths Finder Assessment, Lunch & Learn Webinars, Indigenous Cultural Competency, and 2SLGBTQ Awareness.Social Events: Indwell’s Hope and Homes Banquet yearly, team gatherings.Diversity & InclusionWe are committed to creating a diverse work culture that matches the diversity of our communities.
