Key Responsibilities:
• Conduct thorough interaction checks against full medication lists • Identify and address stale patient data impacting dosage • Assess completeness of clinical information presented • Distinguish between permitted and appropriate medication choices • Ensure clinical conclusions are documented and communicated
Requirements: • Frequent use of Lexidrug or Lexicomp in previous roles • Minimum of 2 years skilled pharmacy experience • Active PharmD and pharmacy license required • Strong knowledge of specialized pharmacy areas • Experience in data annotation or labeling preferred
