You will coordinate services, track progress, and ensure family involvement, playing a vital role in promoting positive student outcomes. Key Responsibilities:
Provide culturally relevant guidance to students and families
Coordinate case management to track student well-being




Collaborate with community agencies for support access
Conduct home visits as part of service integration
Engage with school staff to ensure consistent support Requirements:
Diploma in social work, mental health, or similar
Experience in delivering human services support
Reliable vehicle and valid Driver's License
Solid communication and support facilitation skills
