Complete resident assessments, care plans, and clinical documentation.

Monitor and evaluate resident care plans and make changes as required.

Communicate changes in resident condition to the appropriate team members.

Participate in resident care conferences.

Support resident admissions, transfers, and discharges.

Work collaboratively with the Director of Care and inter-skilled team.

Provide leadership, coaching, and mentorship to team members.

Maintain accurate and up-to-date resident records.

Promote a person-centered approach to resident care.

Qualifications Practical Nurse Diploma.

Current LPN registration and membership in good standing in British Columbia.





Experience in senior care or long-term care is preferred.

Current CPR and First Aid certification.

Computer proficiency and experience with Microsoft Office.

Strong communication and interpersonal skills.

Demonstrated leadership, coaching, and mentorship abilities.

Current Vulnerable Sector Check or willingness to obtain.

Schedule

Current rotation is a 5-on, 2-off schedule, with the potential to transition to a 4-on, 2-off rotation.
