Nurse Practitioner (NP) Community Health & Youth Wellness Location: Scarborough, ON (Community-Based) Employment Type: Part-Time or Full-Time, Permanent Compensation: $52.83 $74.83/hour Prosperity Workforce Solutions is hiring on behalf of a community health organization in Scarborough for Nurse Practitioners (NPs) to join a collaborative, interdisciplinary team providing accessible, client-centered care to diverse and underserved communities.

We are currently recruiting for two NP opportunities: 1.

Nurse Practitioner Primary & Family Care Part-Time: 17.5 hours/week Full-Time: 37.5 hours/week This position focuses on comprehensive primary and family-centered care , including health promotion, disease prevention, and the management of acute and chronic conditions.

You will support children, families, and diverse community populations , including individuals experiencing mental health and substance use concerns.

Key responsibilities include: Provide comprehensive primary and family-centered care within scope of practice Assess, diagnose, treat, and manage acute and chronic conditions Develop evidence-based care plans Provide routine and catch-up immunizations Maintain accurate and confidential EMR documentation Collaborate with an interdisciplinary healthcare team Participate in case conferences, clinical audits, and quality improvement initiatives Support health promotion, community outreach, and program development Provide care to underserved and marginalized populations Conduct home visits as required Participate in an on-call rotation with physician support Support mentorship and education for students and team members 2.

Nurse Practitioner Youth Wellness Hub Part-Time: 21 hours/week This position supports a Youth Wellness Hub serving youth ages 1225 , with a focus on episodic primary care, mental health, and substance use support .

You will work alongside counsellors, therapists, peer support workers, service navigators, and other healthcare professionals to provide accessible, low-barrier,



youth-centered care.

Key responsibilities include: Provide comprehensive, episodic primary care to youth ages 1225 Conduct medical, mental health, and social assessments Assess, diagnose, treat, and manage acute and stable chronic conditions Order and interpret diagnostic tests and prescribe treatments within scope Provide health education, preventative care, and health promotion Support youth experiencing mental health and substance use concerns Collaborate with an interdisciplinary team and community partners Participate in complex care planning and case discussions Support measurement-based care and quality improvement initiatives Provide clinical consultation and guidance to team members as appropriate Develop relationships with community partners and referral networks Qualifications Education & Registration For both opportunities, candidates must have: Master''s degree as a Nurse Practitioner Primary Health Care Current registration in good standing with the College of Nurses of Ontario (CNO), Extended Class Professional liability coverage Membership with NPAO preferred Current CPR HCP certification Experience Experience as a Nurse Practitioner in primary care, community health, youth health, or a similar setting is preferred Experience supporting diverse and underserved populations is an asset Experience with mental health and substance use is strongly preferred for the Youth Wellness Hub position Experience working with children, youth, and families is an asset New and early-career NPs with relevant clinical experience are encouraged to apply What We''re Looking For Strong clinical assessment,
