Rutland UPCC Is Seeking a Family Physician To Provide Primary Care Services Within Its Longitudinal Care Clinic. This Interdisciplinary Clinic Is Comprised Of On-site Physicians, NP's, RN's And Social Workers Who Utilize a Team-based Care Model.



The Patients Who Are Attached To This Clinic Have Primary Care Needs That Include But Are Not Limited To
Chronic disease management
Substance use disorders
HIV primary care
Gender affirming care
