Help vulnerable residents in Simcoe as a Homelessness Prevention Intake Diversion Worker. Be the crucial first contact for families at risk, providing vital support and housing connections.

This temporary, full time position with Norfolk County requires a two-year diploma in a related discipline. You will assess housing needs using the appropriate tools, facilitate access to emergency housing, and connect families to community services. Impact the lives of those in crisis through your proactive approach and knowledge of homelessness risk factors.

Key Responsibilities:
• Provide customer service to individuals facing homelessness
• Assess housing needs with VI-SPDAT or SPDAT tools
• Connect clients to community resources to prevent homelessness




• Facilitate emergency housing program admissions as needed
• Maintain accurate service delivery records in HIFIS

Requirements:
• Two years’ diploma in Social Services or related field
• Over two years’ experience in social services
• Valid Ontario Driver’s Licence with a reliable vehicle
• Knowledge of community services and crisis management
• Experience with data management software and records

Utilize your skills in crisis support and community resource navigation to prevent homelessness effectively.
