Community Outreach & Attachment Worker

Full Time (1.0 FTE) Contract until March 31, 2027

An exciting opportunity is now available for a Community Outreach & Attachment Worker to work as a member of the Haldimand Family Health Team, serving approximately 27,000 patients in Caledonia, Hagersville and Dunnville areas.

The Community Outreach & Attachment Worker will develop and maintain collaborative relationships with municipalities, community organizations, healthcare providers, Ontario Health partners and other stakeholders to strengthen partnerships, promote HFHT programs and services, and support patient attachment initiatives.

The Community Outreach & Attachment Worker will engage with residents who are unattached to a primary care provider, provide information regarding available attachment opportunities, and facilitate attachment to participating physicians and the Haldimand FHT Nurse Practitioner. The Community Outreach & Attachment Worker will also support community engagement activities, communications initiatives and public education that promote the HFHT and improve awareness of available services.

This position is community focused and is not responsible for providing ongoing case management, counselling or system navigation services.

QUALIFICATIONS:

- College diploma or bachelor's degree in Social Work, Social Service Work, Community Development, Health Sciences, Public Health, Communications, Human Services, or another related field, or an equivalent combination of education and relevant experience.
- Bachelor of Social Work (BSW) or Social Service Worker (SSW) diploma considered an asset.
- Minimum two (2) years' experience in community outreach, health promotion, primary care,



community services, or system navigation preferred.
- Knowledge of Haldimand community services and resources preferred.
- Knowledge of the Ontario primary care system, including Family Health Teams, Ontario Health, and patient attachment processes, considered an asset.
- Demonstrated experience building team-oriented relationships with community organizations, health care providers, and community stakeholders.
- Excellent interpersonal, oral, and written communication skills, with experience in public education, presentations, community engagement, and the development of communication materials considered an asset.
- Demonstrated ability to exercise sound judgement, decision-making, problem-solving, conflict resolution, organizational, and prioritization skills while working independently and collaboratively within an interdisciplinary primary health care team.
- Demonstrates independent initiative mindset and creative problem-solving skills.
- Demonstrated knowledge and experience with computer systems and applications. Prior experience and knowledge of OSCAR or another electronic medical record is considered an asset.

· Must have access to a reliable vehicle and possess a valid Ontario driver's license. Salary: $66,000 - $68,000 – dependant on relevant experience.
