About The Clinic

Lifemark Meadowlark Centre in Edmonton is an interdisciplinary rehabilitation clinic that delivers a unique chronic pain program, funded through Alberta Health Services (AHS). The program supports individuals experiencing persistent pain and complex functional concerns through a coordinated, client-centred approach to rehabilitation.

The interdisciplinary team works collaboratively to assess each client’s unique needs and develop individualized rehabilitation plans that address physical, psychological, functional, and social factors affecting recovery. Depending on the client’s needs, care may involve collaboration among physiotherapists, occupational therapists, psychologists, kinesiologists, physicians, nurses, pharmacists, and other health professionals.

The program emphasizes education, active rehabilitation, self-management, functional restoration, and the development of practical strategies to help clients improve their quality of life and participation in meaningful daily activities. Team members work closely with clients and with one another to establish goals, monitor progress, and support sustainable improvements in function.

Position Summary The Active Rehabilitation Nurse (RN) plays an integral role in coordinating and supporting patient care within the Active Rehabilitation Program. This position combines clinical nursing expertise with patient care coordination, interdisciplinary collaboration, program administration, and quality reporting. The RN serves as a central point of communication between physicians, rehabilitation staff, patients, and administrative personnel to ensure timely referrals, accurate documentation, and seamless patient progression through the rehabilitation program.

The successful candidate will possess strong clinical judgment, exceptional organizational skills,



and the ability to manage multiple priorities while maintaining a patient-centered approach.

Key Responsibilities

Patient Care Coordination/Administration

- Review and process physician referrals and rehabilitation orders.
- Coordinate patient intake, orientation, team assessments, and internal consult referrals.
- Monitor patient flow through the rehabilitation program to ensure appropriate scheduling and program capacity.
- Maintain patient caselists and ensure accurate tracking from referral through discharge.
- Prepare and maintain weekly patient census reports.
- Coordinate appointment requests and communicate scheduling information with patients and interdisciplinary team members.
- Monitor patient progression, suspensions, and discharges throughout the program.

Clinical Nursing Responsibilities

- Review patient charts and identify clinical needs requiring intervention or follow-up.
- Complete patient orientations to the active rehab program - group orientation, phone call follow up for orientation packages, consent to participate and day one orientation
- Meet 1 on 1 with patients prior to (or on the start of their program) starting program to assess potential changes since referral and current appropriateness for program.
- Assess urgent patient concerns and provide appropriate recommendations.
- Collaborate with physicians regarding patient care plans, medical clearances, and rehabilitation suitability.
- Communicate directly with referring physicians and healthcare providers regarding patient progress and recommendations.

Documentation & Electronic Health Records





- Maintain accurate and timely documentation.
- Ensure all patient records comply with organizational and regulatory standards.
- Complete assessment and discharge documentation, facilitate sending reports to GP, and ensure appropriate closure of patient files.
- Maintain accurate records of patient attendance, assessments, and program participation
- Assist with CARF accreditation.

Interdisciplinary Collaboration

- Coordinate and prepare weekly interdisciplinary meeting agendas.
- Participate actively in interdisciplinary team meetings.
- Present patient assessments and summarize rehabilitation recommendations.
- Coordinate communication between rehabilitation team members, physicians, and administrative staff.
- Facilitate implementation of rehabilitation plans and follow-up recommendations.
- Support continuity of care through effective communication and documentation.

Qualifications

Required

- Current registration as a Registered Nurse (RN) in good standing with the applicable provincial regulatory body.
- Minimum 2 years of recent nursing experience (rehabilitation, chronic pain, musculoskeletal, community, or outpatient experience preferred).
- Solid clinical assessment and critical thinking skills.
- Experience with electronic medical records and clinical documentation.
- Excellent organizational and time management skills.
- Strong interpersonal and written communication abilities.
- Ability to work independently while collaborating within an interdisciplinary team.
- Proficiency with Microsoft Office, including Word, Excel, and Outlook.

Preferred

- Experience in rehabilitation, chronic pain management, occupational rehabilitation, or outpatient clinical settings.
- Experience coordinating patient care within multidisciplinary healthcare teams.
- Knowledge of rehabilitation principles and return-to-function programming.
- Experience with clinical reporting and healthcare data collection.
