Position #: 9010992 Expected Start Date: October 05, 2026 Union: HSAS Facility: Battlefords Community Health City/Town: North Battleford Department: Chronic Disease Prevention Type: Full-time temporary Expected Up to Date: October 04, 2027 FTE: 1 Shift Information: Days Number of Hours per Rotation: 28 shifts of 8 hours per 6 weeks Relief: No Float: No Hours of Work: Standard Hours Salary or Pay Band: Pay Band Assessor Coordinator - Degree $36.721 to $44.902 Travel Required: Yes : Under the direction of the manager and as part of the interdisciplinary team, the Assessor Coordinator – Degree is responsible for the full scope of case management services. Case management is patient-centered,

strength based, adaptable, cost-conscious and quality driven. One of the key functions of case management is to advocate for and coordinate resources based on assessed needs.

The Assessor

Coordinator – Degree works within Primary Health

Care and is one of the entry points for patients needing access and navigation support to community, home care, long-term care and acute care services. The core functions of case management practice include referral intake, engagement,

care planning, monitoring, evaluation, and termination/discharge. The Assessor

Coordinator – Degree liaises with other services within the network area and throughout the province, in an endeavor to provide the right service at the right time and in the right place.

Human Resources Exemption: No Education Degree in a health or social science related field.

Competencies Time management skills Communication skills including ability to provide educational presentations Knowledge and Abilities Demonstrates effective verbal and written communication skills.





Ability to facilitate applications/participate in integrative case management Knowledge of social determinants of health affecting vulnerable populations Knowledge in client case management and process facilitation Knowledge of crisis intervention strategies Demonstrates empathy, trust and respect in the provision of care Responsible for accurate observation/documentation of patient related info.
