St. Mary’s Home (SMH) is a not-for-profit charitable organization providing specialized community support services and programs to marginalized young pregnant women and young parents (ages 12-29), and their children (ages 0-5). The approach is holistic, grounded in a strong commitment to strengthen and build upon the personal resilience of everyone.
Position Summary:
The Case Manager (CM) reports to the Manager of support services. CM role is to implement the Mission Statement of SMH by providing Support and Case Management to at-risk pregnant/parenting youth and their infants. Within a strength-based philosophical framework, the CM promotes positive dynamics, defuses negative dynamics and is a positive role model.
The Case manager
· Conducts intake and assessment.
· Help with system navigation.
· Refers youth who are pregnant or parenting to programs provided at SMH or at other agencies depending on their needs.
This role provides goal-specific services to SMH participants through participant-centered, strengths-based, and trauma-informed practice.
Responsibilities:
· Maintain comprehensive, accurate, and up-to-date documentation, including record keeping, client files, reports and service coordination and case closure documentation.
· Engage the participants one-on-one in an effective and positive manner and within a Collaborative Problem-solving framework;
· Oversee dynamics within a Crisis Prevention Intervention (CPI)framework;
· Facilitate intakes;
· Implement assessments using approved evidence-based tool;
· Complete appropriate internal and external referrals based on participant-identified needs in their service plan;
· Respond to participant mental health and addiction recovery, family violence and trauma-related challenges.
· Participate in ongoing professional development as required by SMH including but not limited to program training.
· Participate actively in staff meetings.
· Resolution skills building
· Engages in regular supervision and support
· Covers when fellow CM is not available
· Promotes positive morale among team members and agency well-being.
Qualifications:
· Bachelor’s degree in social work or related clinical equivalent
· 4 years counseling / social work experience directly related to addictions, mental health, family violence and/or trauma issues and infant mental health.
OR
· Diploma as a CYW, SSW or related equivalent
· 2 years of progressive experience working directly related to at risk youth, mental health, family violence, trauma and infant mental health
· Minimum 3 years’ experience working with pregnant and parenting youth / At risk youth.
Required Knowledge and Skills:
· Excellent communication and interpersonal skills
· Ability to work independently and as a member of a team
· Capacity to provide service in both official languages. French is mandatory
· Certificate and applied knowledge of Crisis Prevention Intervention
· Certification with the GAIN SS and CANS Assessment tools is an asset, but can be acquired once in the position
· Compassion, enthusiasm and the ability to take initiative
Other duties as assigned.
Pay: $50,000.00 per year
