Your Opportunity:
Reporting to the Program Lead, Clinical Services Integration and Patient Flow Supports, the Consultant is a provincial role that supports enhanced governance, change management, implementation, and continuous improvement initiatives that strengthen integrated primary health care across Alberta. This is a provincial role open to remote/hybrid option that requires working within a highly virtual team environment. Working independently and collaboratively with patients, families, healthcare providers, and system partners, you will support the planning, coordination, and implementation of complex provincial initiatives. Key responsibilities include supporting the Program Lead and Executive Director with governance-related priorities for the Transition in Care (TiC) program; working in a dyad for the design, build, and ongoing enhancement of the TiC Implementation Guide; acting as the department's ABSORB Champion to develop and manage digital learning resources and modules; and serving as the My Learning Link contact for departmental education content and processes. The Consultant applies skills in facilitation, strategic planning, patient engagement, human-centred design, and change management to support the advancement of integrated care and provincial priorities. As part of a passionate team of system transformers, you will support the design, implementation, and spread of creative approaches to care through partnership, co-design,
and effective governance. Key responsibilities include supporting the Program Lead and Executive Director in advancing governance-related initiatives and requests for the Transition in Care (TiC) program; working in a dyad for the design, build, and ongoing enhancement of the TiC Implementation Guide; acting as the department’s ABSORB Champion and My Learning Link contact by supporting the coordination and process management for related digital learning solutions as assigned. The Consultant role requires applying skills in facilitation, academic and business writing, project coordination, patient engagement, change management, and person-centred practices. Required Qualifications: Baccalaureate level degree with minimum of 3 years experience in a healthcare related field.
Experience in planning and implementation in a healthcare organization. Demonstrated pattern of professional and personal development.
Proven ability to work collaboratively in a multidisciplinary team environment and with a broad spectrum of stakeholders (both within and outside of the organization).
Demonstrated ability to foster relationships and partnerships with internal and external partners, including patients/clients, families and caregivers. Demonstrated commitment to and application of patient engagement and Person-Centred Care principles and practices in a health care setting.
Education and demonstrated abilities in change management, quality improvement practices and methodologies, and person-centred practice. Knowledge and demonstrated experience in facilitation or coordination of project management: execution, reporting, monitoring and evaluation. Knowledge and evidence in change management principles with proven ability to foster a culture of change acceptance, adoption and advocacy.
Ability to understand complex health issues through research, analysis and evaluation of best evidence.
Additional Required Qualifications:
Requires exceptional verbal, written and listening skills to comprehend, create and communicate concepts and options to a broad variety of people at different levels of the organization who have different skills and expertise. Ability to develop and facilitate presentations to convey information or transfer knowledge. Demonstrated ability to work independently and with a team within a large, complex organization with a variety of staffing groups.
Demonstrated needs identification, gap analysis, critical thinking skills, decision-making and innovative problem solving. Demonstrated abilities in conflict resolution and management. Excellent computer skills including expert knowledge of Microsoft Word, Power Point, and Excel required.
Preferred Qualifications:
Masters level degree. Formal training and certification in change management, project management and/or improvement methodologies. Knowledge and experience with learning platforms such as Moodle or ABSORB.
