Bridges Community Health Centre (CHC) is a non-profit, community-governed organization funded by the Province of Ontario. Our holistic, interdisciplinary model of health and wellbeing focuses on primary health care, health promotion, illness prevention, and community capacity building, while considering underlying social determinants of health and barriers to accessing care. Our greatest strength is our team, who is committed to the work they do, serving and strengthening the communities of Fort Erie, Port Colborne and Wainfleet.
We are currently recruiting for a permanent, full-time Diabetes Nurse Educator to join our team.
Qualifications Required:
The Diabetes Educator specializes in educating patients around self-management of diabetes in accordance with Diabetes Canada Clinical Practice Guidelines. The Diabetes Educator works in collaboration with a multi-disciplinary team in health promotion and health prevention. Qualifications include:
- Current registration and in good standing as a Registered Nurse (RN) with the College of Nurses of Ontario
- Minimum 2 years’ experience in counselling clients with diabetes on an individual and group basis.
- Certified Diabetes Educator (CDE) an asset.
- Bilingualism an asset
