Key Responsibilities:
• Provide holistic, resident-centered care to the elderly
• Conduct proactive visits and on-site medication reviews
• Attend care conferences and participate in 24/7 on-call




• Work collaboratively with interdisciplinary care teams
• Complete necessary documentation in patient charts

Requirements:
• Experience in family practice preferred
