The Indigenous System
Navigator provides vital support to Indigenous patients, advocating for their needs while ensuring effective navigation through health services. Your focus will be on fostering collaborative relationships and improving health outcomes across the care continuum. Key Responsibilities:
- Act as a liaison between patients and health services
- Coordinate care transitions and discharge planning
- Advocate for Indigenous patients in hospital settings
- Connect individuals with community-based resources
- Facilitate culturally responsive support for patients Requirements:
- Membership in your applicable regulatory body
- Experience in community health or emergency settings
- Strong cultural competency in Indigenous health
- Valid Ontario Driver’s License and vehicle access
- Proficient in health records and database software Support Indigenous health efforts while enhancing patient experiences and community wellness as an Indigenous System Navigator.
