- Minimum of five (5) years of related experience in clinical practice required.
- Previous experience in Continuing Care/ Home Care required.
- Proven clinical and leadership skills.
- Superior organizational skills.
- Evidence of strong leadership,



interpersonal and communication skills.
- Evidence of initiative, flexibility, and creativity.
- Ability to establish and maintain effective relationships with patients, families, physicians, community agencies, professional colleagues, volunteers, and the community.

- Ability to function effectively in a agile work environment.

Preferred Qualifications - Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice will be considered.
