Position #: 9010040 Expected Start Date: September 07, 2026 Union: SUN Facility: Wascana Rehabilitation Centre City/Town: Regina Department: Community Health Services Federal Type: Full-time regular FTE: 1 Shift Information: Days, Evenings, Weekends, Stats Number of Hours per Rotation: 224 hours per 6 weeks; 8 hours shift Relief: No Float: No Hours of Work: Standard Hours Salary or Pay Band: Pay Band Nurse A $38.580 to $50.070 Travel Required: Yes Human Resources Exemption: No : This position will be part of the Intermediate Care Services within the Primary Health Care Networks. This service addresses a provincial health system service gap, where some clients with multiple complex health issues have difficulty in accessing an office-based primary care provider.

The goals for this service are: to provide the most at risk clients with home based access to higher level health care providers, including MDs and NPs; to mitigate the risk for worsening health sates and acute care episodes leading to Emergency department visits and hospitalizations for at risk clients; to reduce system pressures on acute care services, particularly Emergency departments and hospitals; to provide cost effective and high quality health care using patient centered and community based approach. Practitioners in this service are assigned to one of the Primary Health Care Networks and provide support to the interdisciplinary Primary Health Care team. License Registered Nurse (RN) Valid Class 5 driver's license Education Current registration with CRNS and current BCLS certification.

Competencies Critical thinking skills Communication skills Interpersonal skills Demonstrates leadership skills Experience Minimum of two (2) years recent related nursing experience is required Knowledge and Abilities Ability to initiate and exercise critical thinking and decision-making Ability to demonstrate appropriate level of professional conduct and judgement.

Ability to promote effective relationships within the SHA and the community.





Demonstrates application of the nursing process Current clinical knowledge and skill are required in a broad range of areas.

Ability to exercise independent nursing judgement Has demonstrated strong interpersonal and communication skills.

Education and Training Graduate of an approved Nursing school Other Information Home base for this position is: 2180 23rd Avenue (WRC)(Subject to change) Willing to Travel to rural areas as assigned. Travel allowance provided.

Travel required to various community sites within Regina PHC Networks Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS) Must possess a valid driver's license and have a vehicle available for work.
